Sample Employee Opinion Survey

Company: Today's Corporation
All Today’s Corporation employees will be responding to this Employee Opinion Survey.  Peter Barron Stark Companies has customized this survey for Today’s Corporation based on interviews that were recently conducted with Today’s Corporation employees.  The purpose of the survey is to provide management with your opinions about the strengths and areas for improvement within the organization, which will allow them to focus on making Today’s Corporation an even better place to work.

Your honest and candid response to each of the following questions will help to accurately measure the opinions and feelings of Today’s Corporation’ employees.  Your responses will remain anonymous.  The responses will be quantified and presented to you and your management team.  Any written comments will be typed in a collective format to ensure your comments remain anonymous before being presented to management.
Definitions:

Today’s Corporation or Company:
If the question is referring to Today’s Corporation or the Company and not a specific management team or your department, answer the question regarding your perception of the organization as a whole.

My Department:
For questions asking about your department, please refer to your perception of your department or area as a team.

Senior Management Team:
When answering questions about the Senior Management, respond regarding Mr. CEO and his direct reports (i.e., Jane Doe, Mike Johnson, Paul Smith, and Debbie Thompson). If you are a member of the senior management team, answer the questions regarding your team as a whole.

My Supervisor:
When answering questions about your supervisor, please respond regarding the person who directs your daily work/completes your performance review.
Guidelines:

Each response should be based on what you personally know about Today’s Corporation. If you don't understand a question or feel you can't answer it accurately, leave it blank or select "Not Applicable."

Survey responses will remain anonymous and will be averaged with no less than five respondents in each department/category.
